Graduate School Forms

The forms below are all available in Adobe's portable document format  (PDF). To view and print a form, you must have Adobe's Acrobat Reader 3.0 installed on your computer. Simply click on the highlighted name to download and print the form. Early Entry Program Form (early entry undergrad)
To request approval of courses for an early entry student.

Application for Degree
To apply to graduate with a masters or doctoral degree. The "Application for Degree" form may be completed on-line through Banner Self Service.
Application for Graduate Certificate
To apply to receive a graduate certificate. The "Application for Certificate" form may be completed on-line through Banner Self Service.
To request a change in diploma/certificate name, address, telephone number or email address to an already submitted Application for Degree/Certificate. 
To have doctoral dissertation committee appointed.
To report the outcome of a dissertation defense.
Graduate application to establish residency at UNC Charlotte.
To appeal suspension from the Graduate School at UNC Charlotte.
